The HHA is trying to get the insurance ball rolling but can't do it without OUR help!!
It really quite simple and you can help even if you do not use the service.

You answer 6 easy questions, he tabulates the info and we possibly get an insurance program!!
They are NOT asking any questions that would jeopardize your business, and again you can answer without being obligated to participate in the insurance program. The more responses we get the better the chance to get a program in place.
Here's the questions you will be asked:
1.) Gender -
2.) Date of Birth -
3.) Zip Code -
4.) Select One Coverage Levels -( Individual, Employee & Spouse, Employee & Child or Family)
5.) Risks- Have you incurred $10,000 or more in medical expenses in the last 12 months or do you expect to incur $10,000 or more in the next 12 months?
6.) If Yes for question # 5 please provide a brief description of the diagnosis and treatment plan.
See...no attendance totals, no price structures, no SS # or other non-pertinent questions, not even your name! Allan has volunteered to do all the hard work, all you have to do is email him and answer the questions! SIMPLE!!
